Affected Groups:  Those who use APS forms

What is Happening?  We are launching a new electronic process for Authorization of Professional Services (APS) forms.  All APS forms will be completed using Adobe Sign to maximize efficiency and eliminate the need for paper based forms.

When Will This Happen?  Electronic APS forms are now available for use with Adobe Sign.  No other methods will be accepted after Friday, January 28, 2022.

What Do I Need to Do?  Preparers/senders can access four templates stored in Adobe Sign to use a prebuilt process.

APS forms are not available for download

There are four templates for APS forms stored inside of Adobe Sign.  Preparers need to follow the steps below to access the templates.  A video tutorial for each template is provided.

How to Use APS Forms

  1. Review instructions
  2. Complete the Adobe Sign course
  3. Decide which template fits your use case and watch the applicable tutorial
  4. Search for the template within the Adobe Sign application
  5. Prepare and send the form using Adobe Sign
  6. Email completed forms with all signatures to aps@uta.edu

Please Note

  • Instructions for requesting individual access and user groups are provided in the training
  • Recipients/signers do not need access to Adobe Sign or UTA credentials to sign APS forms.
  • The final signature has changed from the President to the Associate Vice President for Business Affairs and Controller (Stephanie Scott).
